These documents will assist you with FOCUS survey buy-in and completion in your department.

Demographic Form Guiding Document

This document lists the information needed to complete the Demographic Form, which is required before you can begin the full FOCUS survey. We recommend familiarizing yourself with this document and gathering the necessary information ahead of time before filling out the Demographic Form.

Labor-Management Letter of Support Template

This document should be signed by a management and union representative in your department to show your membership that the survey has mutual support at all levels across the department.

Mental Health Resource Guide

This document should be given to all members during survey administration with the following language:

"This survey asks questions about sensitive topics. Because it is anonymous, the FIRST Center will not be able to provide us with any individual scores or referrals. If you are concerned about your mood, please refer to the Mental Health Resource Guide the FIRST Center team provided to our department."

Please add department-specific resources as appropriate.

Posters

Physical copies can be hung up around your department as encouragement for rank-and-file who may be hesitant toward the survey. Digital copies can be added to email, document, or assignment headers.
